Problem at Main Menu

I am trying to test the component. When I try to add new item at main menu (i.e. Categories) the next screen is blocked.

I am using Joomla 2.5.28...

Any suggestions?

can you send me the php logs when you load the page?

After moving to a new server we still have the same problem adding "Categories" at main menu. The error at log file shows:

Any idea?

I don't know libraries/expose/admin/fields/imagelist.php
Is it a joomla extension?
